The Sub-Acute Care program at the LaChance Center provides an effective response to an emerging medical need for patients of all ages. As patients are discharged from acute-care hospital settings more quickly, allowing for more efficient care and shorter recuperative periods, families and patients are looking for care outside of a hospital setting.
  The LaChance Center Sub-Acute Care program allows patients to ease their transition home by providing specialized and personalized nursing care and rehabilitation. Rehabilitative Services, including Physical Therapy, Occupational Therapy and Speech Therapy are available on-site and are tailored to each patient's needs and the projected length of stay. Our goal at the LaChance Center is to help each patient reach the highest level of functioning possible prior to returning home.
  Our nursing staff, admissions team and discharge planners, will work with hospital staff and physicians to ensure a smooth transition from the hospital to the LaChance Center.
