Our comprehensive nursing care program provides the foundation for quality patient care. Skilled professional nurses assess and evaluate all patients and communicate regularly with referring hospitals, physicians, HMO'S, families and discharge planners for optimal care, planning and outcomes. This skilled department delivers excellent medical care including all forms of IV therapy, wound care, skilled monitoring of complex medical conditions, and respiratory interventions utilizing an interdisciplinary approach. These may include the rehabilitation department, dietitian, social worker, psychiatric services, activities, as well as spiritual support.
Whether patients are with us for long-term or short-term care the goal of nursing is to provide highly skilled care in a comfortable, supportive, and caring environment.

Long Term Care

The LaChance Center has proven expertise in long term care providing for
individualized needs through one of our three Long Term Care Units.
The Long Term Care Unit's home like atmosphere can accommodate residents
of all ages. Every member of our staff understands and respects that the center
is the home of each of our residents. Our residents' needs for privacy,
dignity, safety and comfort, provide the framework for how care is
delivered.
At the LaChance Center, the quality of life each resident enjoys is as
important to us as the quality of his or her care. Through nursing and
interdisciplinary team assessments, each resident receives extensive
evaluations to create an individualized care plan. Residents and families
are a part of this team allowing residents as able to dictate their needs
and wants, at the same time giving those whom require more supervision
gentle cues and redirection. Each resident's physical, psychosocial and
emotional status is continuously monitored to assure that each is living his
or her life to the individual's ultimate level of functional independence.
