The Legend Center has proven expertise in extended health care services, providing for individualized needs through one of our three Long-Term Care Units.

The Long-Term Care Unit's homelike atmosphere accommodates adult residents of all ages and medical conditions.

Every member of our staff understands and respects that the Legend Center is more than a medical facility - it's the home of each of our residents. We put the highest emphasis on our residents' needs for privacy, dignity, safety and comfort.

Through nursing and interdisciplinary team assessments, each resident receives extensive evaluations to create an individualized care plan. Residents and families are a part of this team, allowing residents, within their abilities, to dictate their needs and wants.

Each resident’s physical, psychosocial and emotional status is continuously monitored to assure that each is living his or her life to the ultimate level of functional independence.

Nursing Care Service
Our comprehensive nursing care program provides the foundation for quality patient care. Skilled professional nurses assess and evaluate all patients and communicate regularly with referring hospitals, physicians, HMOs, families and discharge planners for optimal care, planning and outcomes.

This skilled department delivers excellent medical care including all forms of:

  • IV therapy
  • Wound care
  • Skilled monitoring of complex medical conditions
  • Respiratory interventions


Our staff’s interdisciplinary approach embraces the full scope of your medical care. Our treatment and assessments include input from your:

  • Rehabilitation team
  • Dietitian
  • Social worker
  • Psychiatric services provider
  • Activities leaders
  • Spiritual support groups

The Setting
Legend's Extended Health Care Services home-style environment provides an Eden-like setting to mitigate against the issues that too often arise in a long-term facility, such as boredom, loneliness and helplessness. Our residents are part of the Legend community, coming together to share experiences and support.
