The LaChance Center sub-acute care unit provides a unique healthcare environment for patients of all ages who no longer require the full services of an acute hospital, yet whose needs are too complex for a skilled nursing facility.

Our elegant facilities include a newly designed dining room, comfortable patient rooms that are well-appointed, hair salon, chapel, spacious patio gardens and varied activities.

Our founder, Leo P. LaChance, successful businessman, real estate developer and philanthropist, created Eastwood Pines Nursing Home in 1969 when he recognized the need for quality, state-of-the-art nursing care for his mother. As healthcare and rehabilitative technologies have changed, Mr. LaChance has expanded the realm of services.
Recently, extensive facility renovations were completed and a state-of-the-art sub-acute care unit was added. Eastwood Pines has expanded its focus in the areas of skilled rehabilitation and sub-acute nursing services.
We continue to provide high quality long term care for that segment of our population. The newly designed 120 bed facility changed its name, along with its service offerings, and is now known as the
"Leo P. LaChance Center for Rehabilitation & Nursing".
